Columbia, SC—April 23, 2014—With funding from the Centers for Medicare and Medicaid Services, the Office for the Study of Aging engaged the Arnold School of Public Health Office of Public Health Practice for technical assistance with database development, electronic survey development, offline web application programming, and custom form design for participant enrollment and tracking.
Nurses use the tool during home care visits to consent and enroll participants, perform baseline and follow-up assessments, track medications, and maintain a plan of care for participants over time. It allows medical and research personnel to identify and address issues before they require a transition in care for the patient, thus reducing costs and helping to achieve desired outcomes for patients. Site features include data input validation on all forms, custom reporting, date-based alerts for each patient, and offline access to the system when an Internet connection is not available in the field.
